Spin on transmission filter fell off

So yesterday while heading home for lunch, I notice that my truck really doesn't like shifting 1-2, 2-3 or 3-4. In many cases, it acted like it would rev up, then go from 1st to 4th, followed by a CEL. Pulled codes and got Gear2-4 ratio and Low trans pressure codes (4x codes).



I decided the easy fix was to check trans fluid: Full and clean



Then drain the trans fluid into a new and clean 5gal bucket, so I could reuse.



I dropped the pan and quickly discovered the spin-on filter was laying at the bottom of the pan, along with the threaded adapter. How the heck could that just fall off?



Went out and bought two new filters, replaced, put everything back together and no more issues.

The simple answer would be it was not installed properly. The other thing is if it was overtorqued you may have cracked the mounting nipple in which case it will come off again

I'd like to take responsibility, but I didn't install the last set of trans filters. With that being said, they were on there much longer than they should have. . Easily almost 40k miles based on my records, so they needed replacing anyway. Still concerning to see it just laying there. I'm wondering if my offroad excursions are too much for this motor/trans and causing this stuff. .

None of my friends that off road with me have had this happen ... ... ... ... ... ... ... . yet at least
