My '06 with 48RE developed a driveline vibration when it had only 135k miles on it. It was speed related and occurred only around 62 mph. I was hauling trailers commercially and on the west coast when the problem showed up and tried to continue working for a few days until I could get it home and diagnose the problem.
The problem quickly got worse and began dumping transmission fluid out of the tailshaft and forced me to abort a trip and get it home. I had to take it to an unfamiliar dealer in Flagstaff, AZ to have a new tailshaft seal installed so I could keep fluid in it long enough to nurse it home.
The aluminum sleeve bushing that the tailshaft rides in had simply slipped out of the tailshaft. It is supposed to be pressed in snugly. An apparent machining problem or installation problem with the sleeve allowed it to slide out of the tailshaft. Without the bushing in place the tailshaft was free to wobble and it quickly wore out the tailshaft seal and allowed transmission fluid to leak out.
I had the transmission pulled by an expert rebuilder who performed a full DTT upgrade with all the heavy duty/high performance billet upgrade parts.
The tailshaft will have to be removed from the transmission to replace the bushing if that is your problem. I don't know if the tailshaft can be removed while the transmission is still in the truck.
