I have not been driving the CTD very much lately, but I have these two codes that keep coming up. The CTD will billow smoke uncontrollably and the revving of the engine on its own, goes from about 1200 rpm to almost dying all by itself and sometimes does die. So, I shut the CTD off and then restart it about 20 seconds later and it runs fine again. A little smoke comes out but it quickly clears up. Do I need to replace the cam positioning sensor and do something about the low speed fan relay? Is this just because I don't drive it but a few times a month? What do I need to do? Any help would be appreciated. Thanks
 
Take a look at the lower passenger-side wiring harness that runs along the radiator on the inside. It's very common to have wires chaffed in there, which can cause all manner of strange behavior. It would explain your 0480 (and possibly the 0341).

If there's no damage to that wiring harness, you could have a bad fan clutch. To test it, try spinning the fan by hand. Give it one good hard shove and if it makes more than 4 complete revolutions before coming to rest, the clutch is shot.

Finally, your rough running and revving of the engine, if not caused by some issue with the CPS, could very well be bad injectors.
