wheel lugs in frame?

Just got a "new to me" 06 last week. The bed(before I got it) had ZERO scratches on it. Anyway, I was installing my fifth wheel hitch today and saw these. They look like lug bolts and they're in the frame. I thought maybe this truck has had a flat bed on it and that's why the regular bed was so nice.

Anyone seen this before? How would they be put in thru a boxed frame? They also line up for fifth wheel hitch brackets.

If they were factory, they were installed wile the steel was still flat. then they bend it weld it then hydro form it. I install hitches in my shop and have never been that lucky.

They could have been installed aftermarket. Drill a hole, tie a fishing line to threads, pull bolt thru hole & seat bolt in hole with a nut and washer!
