My hitch is dead center above the rear axle. I've heard of people putting them an inch or two forward the rear to help get some of the wieght toward the front though. We just keep all of our heavy gear in a bed box in front, guess it helps to do the same thing.

Exactly 3. 5" ahead of rear axle goes the king pin. If I sit on the hitch such as when I'm digging through my truck box, that's the exact location of me balls.
 
a buddy has a 5th wheel and his hitch can be slid back something like a foot just by pulling a lever. That way he can have a short box and whenever he gets into tight spot he just jumps out, pulls the lever and presto-change-o the trailer is then back far enough to turn real sharp. Once he's ready for the open road he can slide it back over the axle. nice setup for short box'ers like us.
