Engine/Transmission (1994 - 1998) Who's having luck w/ the upgraded 5th gear nut? Or just get the splined shaft?

I posted here due to you guys having much more NV experience. I have a drivetrain I'm using that came in my 92 W250, it has an NV4500. I know from going through the previous owners paperwork that he purchased the upgraded 5th gear nut and installed it. While the trans is out, is the splined shaft a worthwhile upgrade? What else should I do? This truck (93 W250) is going to spend 90% of its life towing nothing, occasionally I'm going to pull my open car trailer. Its going to have about 350-400 hp also. Any help would be appreciated-Nick
 
NV4500 main shaft

I run 800 fp torque, and knocked the 5th gear loose. Went with a Blumenthal main shaft, which has a full spline under 5th. The stock gear has a full spline, it is the shaft. Also, used their lock nut, which come with sleeve lock, set screws, and the back of the nut punched into a channel in the shaft. Three year and pulling a 15k trailer, no problems with 5th gear. If you have the trans out, might as well do.

I've put over 100,000 miles on the upgradeed nut and washer with a new gear with no problems. I pull heavy loads, gross truck and trailer weight in the low 30k range.

I put the new 5th gear nut on and run it over 150K and its done good so far. But as I am now in the process of yanking my transmission to rebuild it, I'm going to order the silver preimum max rebuild kit from "Quad 4X4" in Montana and that comes with all new output shaft and gear ect... The transmission will now outlast the truck.

I've got most of the tools to do a rebuild on one. If you need to use them let me know. I've rebuilt several and have had no trouble with the fully splined mainshaft with the updated nut. I have never torn into one where the existing mainshaft was what I felt to be usable. If it's got the orange crud around 5th gear, then it's loose and I would replace it.
 
I lost fifth gear on my 98, 24 valve at about 170,000. I tow a 35 foot travel trailer on about 3 to 4 trips a year. I had my dealer replace with the revised nut about June 06 and have had no further problems.
