Why is "high idle" not standard?

There has been discussion about this feature, but I have not seen any explanation as to why it has to be activated by the dealer,... rather than simply standard from the factory.



Are there reasons why some (or most) would NOT want high idle?

I would imagine it could be a profit generator for some dealers.



My dealer didn't charge me, even though I was quoted $40. 00 to turn it on when I called and setup appointment.
 
The main reason for high idle is to run a PTO.

I would not want it, because it takes me 5-10 miles to warm up my truck (or about half a gallon burned), so I would rather get somewhere, than burn the fuel at fast idle.
