Most auto parts should have it available. I believe it's DORMAN Part # 04606, Pioneer automotive part #FRA533, if you want to go with a stock replacement.
Upfront, I know almost nothing about A/T's and such, BUT that center hub crack to me needs a little bit more info. Any history of ANY trans R&R on this truck? In the clutch world, this is an alignment problem. It caused flexing and flexed the flex plate until failure. Condition of torque converter pump hub and front seal, OK or torn up?

The old Mopar/ Direct Connection "speed bibles" told you how to check for alignment. The manual is done just as Gary said. But the automatic is similar too. The snag is that you have to gut the case so you can work "through" it.

They should all be the same. PATC has the 4 mil plates at a decent price also. Gives a little more beef on the flywheel.
