Meh, if you have $550 burning a hole in your pocket go for it. Definitely will give you a boost in power. Highly doubt it will increase MPG. I've bought soooo many modules and programmers for 6 CTD trucks and tried multiple on some trucks, never, ever seen a mpg increase over thousands of miles.
 
Something to consider is pressure boxes like this are harder or injectors. Might not significantly decrease the life expectancy but will increase the erosion on the holes.

I ran a Smarty on my 06 5.9 and kept it on the 60 HP setting most of the time. I consistently saw a 1.5 to 2 MPG benefit vs without the Smarty. That was not measured by the truck but by my hand calculations.

Smarty adds timing even in the 60HP. Depending the actual Smarty model it could also add pressure. Most of the efficiency improvement is from the additional timing though.
 
The even number programs on the Smarty dont add timing.... the odd ones do. It was most relevant when stacking Smarty on top of Edge or TST on the Gen 2 and 3 trucks.

On my 09 4500, I initially bought the Smarty 6.7 to increase the speed limiter from 76 MPH to 95 MPH, but never ran over 80 MPH solo. I ran level 5 (90HP/180TQ) 8.5 years. I deleted it with the Smarty Middle East tune for about 7 years, reinstalling everything about a year before I traded it.

That Smarty allowed custom tuning, but i really wasn't interested in learning enough to do any better than the regular tunes.

BTW, I have a similar product that OP asked about currently on my 2017 Ram 5500 set to 60HP/120TQ. I really do feel it, and I reason that if my mileage DOESN'T drop while using that extra power, it's a mileage improvement, otherwise there would be a mileage drop using more power.

I bought my module in 2018 and like it. Feel it on the bottom and middle, where I typically run this truck.

I dont post about much cuz there's a bunch on here who don't believe in modules. Smarty was a great product, but had issues with the Italians getting back with me, then reading through their website.found they didnt support 4500/5500 trucks. Then someone pointed out where one location on their website said they do and another area said they didnt.

I got the power I want for $500, and with Smarty, the programmer, the unlock, and other mess was somewhere around $1200 to $1500.

Anyhoo, That the WHOLE story.

True, until you adjust the revo settings.

I was pretty much paraphrasing Smarty instructions... and I could see when went in started altering timing, fueling, etc, but then it really wasn't the standard tune anymore.
