Additionally, inside the tank I hear (and can see) some activity and spray that is almost exiting the fuel filler tube with a 3/4 full tank...

    YES - all filters / canisters were refilled with fuel. The cavitation is like an oscilation that started like a loud scream (assumed no fuel in pump) and then after cracking the separator I get the cavitation. I crack the separator because it is the only thing I can do to get the pump to do...
