Engine/Transmission (1998.5 - 2002) '01 won't get over 1500 rpm?

My friend has on '01 and everyonce in a while it will not rev up. It is sluggish. It will blow a little smoke out while it is doing this, but all at once it will clean up and GO! Any ideas?

Any mods on the truck? I just saw another comp with a bad MAP sensor plugin that gave the same issues. Fuel pressures O. K? , Check engine light go on when it happens? , Tach working / not working when condition occures? There's ALOT of variables to go on but more info is needed.

I've seen the same symptoms with a dead map sensor.

I had something like that with an injector pump. Nothing like bad news... . :{
