No they didn't have one but all the mounting holes are there. I bought one for an 04 and installed it in mine. I was trying to make it as quiet as possible.
That's a good question, and I think the answer is "maybe", but probably not in any significant way. Mine is 7 years old now, and shows no signs of paint damage due to heat.
I think there's enough airflow under the hood to keep the metal cool. And at idle you're not generating much heat anyway.
I assume they added the pad in 2004 to quiet things down, but maybe I'm wrong.
