In the fine print it says "reproduction of this form is not acceptable"
I needed the antifreeze anyway (no forms in the store), bought 4 gallons and sent in the forms that I downloaded. Will I get 1/2 my money back? Who knows, but thought I'd pass on the info to those that are planning on changing their coolant.
