My copy of the 93 Dodge Service Manual shows
specified lug nut torque for duals as 325 ft/lbs on a 5/8x18 stud using an 1 1/8" flanged nut on pages O-30, 5-65, 22-6, and 22-13. I've never had the nerve to put that
much on it yet. I use a 3/4" drive ratchet with a 16" pipe slid over it to turn a 4" extension on an 1 1/8" socket, but I don't think I'm putting much more than 200 ft/lbs on the lug nuts. How tight do you go?

Torque them to 350ft/lbs, and don't worry about breaking them. They will come loose if you don't have them tight #ad

Mel Fueling, you could also use a torque stick from snap-on if you have a big enough impact wrench; 3/4 would work well. The yellow one is 350ft-lbs.

i use a 2 piece lug wrench,
a cross type breaker bar is about 2 '
i pull it hard until i cant and it stops
to break loose i stand on one end of the bar
ide guess they r tight, never came loose
i friend w/ furd 350 tow truck, has
had 2x rears came loose 1 time his rear
tire passed him,
ide guess he does not tighten them enough
or ford has a poor design.
mine is a 3500 duelly

It would be ideal to install 2 or 4 of the 5/8 thread 3/4 ton lug nuts first to moderate torque so their cones would help center the wheels. Install four of the flanged dually nuts and tighten. Remove the other lug nuts and install the rest of the dually nuts.

The flange design means a lot of friction and the extra torque is needed. The 3/4 ton nuts use only around 200 ft lb in comparison.
 
Tsb 22-02-92 addressed the problem with centering the wheel on the wheel studs.
The Dodge dealers would provide(no charge) 1st gen truck owners with two cone nuts to center the wheel. You would tighen the two cone nuts first centering the wheel then tighten the 6 stock nuts remove the 2 cone nuts and put on the 2 remaining stock nuts.
