Sounds to me like your clutch is not disengaging fully. I don't know if the point of engagement is adjustable or not. If it is hard to get into just one gear, it could be the synchronizer(s).
 
Try changing the lube. Mine started getting stiff going into 3rd at 74K (did first castrol change @ 32K). Changed to Amsoil series 2000... took 500 miles, but gradually disappeared. No more problems for 7K miles

I have a 95 CTD, 5-speed, 4x4, club cab, 134,000. The transmission shifted like a hot knife through butter when new. Now it wants to stick in gear and go into the next gear hard. Still have 5th gear but expect it to go any time. Clutch master cylinder and transmission fluid levels check OK. Towing is light to 8,000 lbs. on an infrequent basis. I can't tell if this is a clutch or transmission problem. Anyone got any ideas?
 
Try shifting it with the engine off sitting in your drive way. If it will shift to all the gears easy then you have a cluch disengagement problem.

Ditto on that! I ended up short when I went to change out all my gear lubes with Amsoil 2000. Had the wife pick up some Castrol GL5 for the transmission. Figured I would run it until I could get more of the 2000. After discussing it with other members, I relaxed a bit, well too long it turned out. Even though I had the 2000, because my outdoor garage was a bit to drafty, I kept putting off changing out the transmission fluid. First off, with the GL5 Syntorq in, shifting to second while cold was hard. Then, I was noticing it was getting hard to shift period. Changed out to Amsoil 2000 yesterday and the transmission is shifting good again! I'm a believer!

Amsoil is your best choice for lube but check the clutch. They get old also. Mine went at 166K with the same symptoms.

Your miles are close to about the time.
Have you ever pushed down the clutch and not gotten it to disengage?? Or maybe once in the past you thought maybe you didn"t get the pedal down close to the floor board??
One major problem with hard shifting not covered on the trouble shooting charts is the begining of the total failure of the in-put shaft pilot (caged-open needdel)bearing.
Try this . If you cant move between gears--pull over th the side ,shut down eng-then move the gears from 1-2-3-4-5. in any order
Now restart eng and setting still try the same again. Now it wont move gear to gear,with out pumping the clutch??
Pilot bearing about to bit the big one. When this happens one needle cwrals up ontop of the detieraited cage and other needle and the input shaft locks into flywheel and NO clutch disengagement! no release. Shut off eng and again you can slide the stick from gear-to gear. Change the lube,brand,- but what you described think you best call So. Bnd. clutch and get a bearing/bushing that won't ever do this again...
