5 speed transmission shifter problems

My neighbor picked up a 91 diesel 2wd truck with a 5 speed (Getreg?). Occasionally, the shifter gets jambed up in between gears. It did it enough in the past, the previous owner cut some of the floor out, so the shift cover could be removed thru the floor. The neighbor pulled the cover off and brought it to my shop last night. I noticed the 5th/reverse shift fork has been broken at one time and re-welded up by the shift rail. Also, when removing the shifter, there is only one bolt that holds the shifter into the cover. I am thinking a different 5th/reverse shift fork and another bolt to hold the shifter, it might work alot better. I do not see any abnormal wear on the shift rails or the bottom of the shifter. Where can I pick up some parts for this transmission? Thanks.

I think that Standard Transmission in Dallas TX & Zubrota Gear in Minnesota ??? has those also.
