I am going to replace my clutch soon. How do I check the 5th gear nut/thingy to make sure it's OK while I have the transmission out.. 240,000 miles on transmission
 
that problem may have been solved by the time your truck came out. I have over 500K on an NV4500 in my 99 and have not had the problem.
The gear is external to the case and can be seen by removing the tailshaft housing on the 2wd.
 
You will need to seperate the transfer case from the transmission

Just had the 5th gear quit in a 98 and 4th is still pulling fine. How long can I drive this?? I need to get home from Wash State to Wyoming. Can I do this or should I get it fixed?

Been in these before and I think this one has the full splined shaft but I think the nut has come off and the gear has slid.

I think I'd drop the transfer case and see what happened before going that far if for some reason the gear or nut is broke it can make one he!! Of a mess if nothing else drop it pull the nut and the top gear off the shaft and put it back together
 
NV never re-designed the mainshaft to spline it under the full length of the gear. It is generally the case that you should go to the fully splined mainshaft.
