9,000 miles ago my 5th gear went out and was repaired by Dodge. Yesterday it went again, any members out there know of any shops in California or possibly Nevada that are capable of doing a permanent repair.
 
No shops that I know of around here that would give you a deal and a reliable repair. But I would call Standard transmission(I think that is the website name as well). They have great prices on NV4500s that have been rebuilt with the fully splined main shaft and fifth gear nut. I believe they even have a deal on a transmission with the bigger input shaft along with everything else to make it extremely reliable.
 
Without the fully splined main shaft modification the 5th gear nut will not stay on. Mine came off 2 times in 10,000 miles and after the mainshaft replacement it is still good 35,000 later.
 
I had my 4500 replaced by the Dodge dealer in Nevada City, CA last week. I ordered the transmission from Standard Transmission for $1650 plus 150 freight. Took three days. Dodge charged me $1100 to install. Truck was disassembled by Dodge and they quoted me $5,000 plus tax to install a Dodge rebuilt transmission with the same crapy shaft. I got great service from Standard.

$1100? I thought a manual trans R&R was more like $400. That's the quote I got (not from dealer), including clutch and rear main seal.
