I have a 93 w250 with a 94 transmission I was having problems with the truck charging so I put a voltage regulator in it started right away we drove it to the tag office to get it tagged went out for lunch when we went to start it all it would do is Click very loudly we had it towed home took the starter out and had it tested starter tested good put it back on took the battery out had the battery tested they tested it bad at 67% I knew it was good but I bought a new battery put it on and click nothing took off the battery went and bought the starter put it on loud click we've looked at wiring make sure nothing is obviously wrong check fuses we are at a loss please help

Check the fusible links on the driver side fender well. The one that powers the starter relay may burned mostly thru. The relay could also be bad on the fender.
 
And it's a long shot since it ran fine when you shut it off, make sure you can bar the engine over to confirm it isn't locked up. Happens more often than you would think on gas rigs.
 
You took off new battery. Did you put the new one back on? Or did you manage to keep the old one and reinstalled *it?*
Don't know much about 1rst gen, does it have pdc (power distribution center,fuses,relays in it.) If so check the power and ground that hook up to the pdc,on 2nd gen. Some members have had those connections work lose.
 
Did you ever get your truck running?*
We over looked a key item is alternator working? *Voltage wotking?
Of course when you put a new battery in it should of turned the starter over.
