Most likely a bad injector, but could be a transfer tube o-ring.

If the level is real high you might need to have someone with a bore scope check the pistons/ cylinder walls for damage.
 
Last time it did this (last august), we put new injectors in and that seemed to cure the problem, as it also had a clogged injector or two (or symptoms indicated it).



Do you think a new Oring on the injector would fix the leaking problem? It shoulnd't have a problem within the cylinders, just above them (i'm guessing)



Also, the line in the back of the head- can it have any pressure on it (have a remote mounted FPR on the truck) or do we need to run a line from the back of the block to the tank? I assume its the extra fuel used to cool the injectors, correct?
 
Also, if its any coralation, we just got fuel pressure back into the system (19psi at idle, 18psi at WOT with box on 5x5 and 275 injectors). We hadn't noticed it before, but since the last 50-100 miles, its gained a quart of fluid in the oil pan :--)
 
Well, just a WAG, I'd say you have a bad transfer tube. If this condition existed before you did 275s, went away briefly, now is back?????

Did you make sure the tube seated correctly after the new injectors?

I'm not a "wrench" but I've been there done that. Mike
