I find that while I am under the hood attempting to break (fix) things that I catch one of the vac lines with a wrench or screwdirver and pull it out of its connection. They slide out so easy you dont even no you did it until you try and operate the AC/ vent controls... ... ... . My brother-in-laws 96' was notorious for this. The rubber connectors and t-connectors were just worn out and the vibrations would just rattle them apart. Most common connections that I have found that get disconnected are those that run left to right just above the valve cover and just below the rubber seal for the hood towards the firewall ... ... .
