Belt Tensioner Problems?

Has anyone ever had the drive belt cut into the belt tensioner? I just changed my belt tensioner and I would say that a 1/2 inch or so of the belt was gone. Upon inspection it appears that the belt has sawed away some of the metal of the tensioner. Has anyone seeen this before? After I got the new tensioner on and the new belt on it looks like every thing is running smoothly. I just bought this truck a few months ago and I didn't notice it then. The pulley could spin freely on the old tensioner but not the new one, so I would say the pulley bearings were gone for a while.

yea iv replaced a couple of them. Mine is getn bad now on the 93 im driving. I think they wear on the pivot bolt over tome and get sideways then the belt slides forward. i was just thinkn bout goin to napa to get 1 this morn. Wish i saved the cummins part # from the last truck wen i replaced that 1.
 
I cut 2 bran new belts before I realized it was the tensioner causing the problem. Replaced it and haven't had a belt problem since
 
I've seen it lots of time on heavy equipment. There is a plastic bushing in the tensioner that wears out, letting the tensioner lean out and cut the belt on other pulleys.
 
I say use a Cummins belt tensioner ONLY! I replaced mine when it made noise, all has been well since, about two-years ago.
