I have to codes showing PO514 and PO628 on my 2004 1/2 3500 6 speed. Truck has shut down twice and had to wait a few minutes to restart any ideas?

0514 is nuisance thing that can be stopped with a SW update - or ignored.
0628 means your lift pump has had it. Don't keep pushing it or it will leave you stranded and could even hurt your HPFP. Either install the in-tank setup (my personal choice), install another canister mounted pump or go after market.
