I have a 95 truck with a NV4500 and I am giving though to putting in a auto trans. I just turned 75 years old and shifting is a pain.
I have a 518 trans that is new built with some extra goodies in it so it will hold the power but I need to know if the adapter on the
motor that has worked with the std. bell housing will work with the auto bell housing. If not will a adapter from a early 89 or 90
that used a 727 trans work? Also will a flex plate from the 89 or 90 work with the later converter? Any help will be greatly appreciated

The number of people who want to switch to a manual trans is a lot greater than those who want to switch to an automatic. I suggest you find one of those guys, preferably a 94 or 95 so your PCMs will swap, and work out a vehicle to vehicle switch. Even easier, just trade trucks maybe. To use the 518, I can't help you.
 
I've got a 95 3500 with a brand new rebuilt Jasper in it (ok, it's got maybe 500 miles on it since it was put it) and I'm looking at having to yank it for a rebuild just to take additional power when towing. I hadn't considered the notion of going to a manual but that makes a lot of sense.

How much of a PITA would a swap be?

If you have the donor truck right next to you and you have some mechanical skills and the essential tools, not that bad a job. Assembling all the parts and not knowing exactly what you need will make it much tougher.
