Which Exhaust Brake for HO w/48RE transmission

I just got back home from a trip through New England with my 2003 HO with 48RE pulling a 33 foot trailer. Three things happened that I don't understand.



1. When I first started the engine in the morning the throttle was dead - pushed to floor and nothing happened. After a minute or so it was okay. This has happened 8 or 10 times.



2. When pulling up a hill, overdrive off at about 2700 rpm, the rpm would go to 3200 and then return to 2700 again.



3. When pulling up a hill at full throttle it would start jumping so bad I had to let off the throttle. This happened twice. I had some trouble with this on my '95 also before they reflashed the computer.



I love the truck. It is quiet, rides great looks good. Mileage is 10-11 when pulling the trailer and around 18 empty. My '95 was 11-12 when pulling the trailer and 21 empty. It has a little more power then the "adjusted" '95.
 
A possible answer to question #1, it is a drive by wire truck so just electrons making things happen. On my brothers big truck it is programed so that you can't do anything when you first start it when it is cold out. Is it possible that is an active feature on the newer trucks?
