my brother says that some 3500's came from the factory with an exhaust brake. I don't think they ever did. Who is right?

Not that I've heard of. Maybe the dealer put one on and then sold it.

The Jacobs is a dealer-installed option that carries a Mopar accessory part number. I don't think any were factory installed.

It is a dealer option, so yeah and no. It all depends on how you look at it.
