What gears should it work in, mine is 3rd & 4th only.

sounds right mine cuts out around 25 mph. It will run at a stop when cold enough.

I'll have to double check mine, but think it works at least in second also. It will turn off at 20mph just not sure what gear it is in.

It should work in second as well. The trans will automatically downshift from 4th to 3rd at about 50mph and re-engage the brake. To get second you have to use the shift lever and manually select it. The brake should engage and slow the truck to about 22mph before shutting off.
