I have all of the TDR issues back to 1995 and I need to know if there is a guide to re torque the fifth gear nut in the truck, I think I have it figured out but some light reading I can co before I start

Are you asking what you need to do to fix the nut or whether you need to open it up and re-torque it to keep it from coming off?
 
It has come off( no 5th gear) and so I need to pull T case and then the back "add on" compartment that has the 5th and reverse gears and clean and replace 5th nut and I have the spline over nut fix from the guys in Denver that goes into the nut and is captured to the shaft splines so It won't let the nut back off. That is where I am starting now.

Throw the splined retainer in the garbage it still lets the nut back off just a little bit and then you will chip gear teeth . Been there tried it . The best luck I’ve had holding the nut on is green sleeve retainer on the internal splines on the gear and shaft splines then get your choice of nut and more sleeve retainer tighten nut to spec let the loctite setup for a day or two and go back to work
 
I am on my 2nd NV4500 in my 99 CTD and had the first one out at 100k to put a clutch in it and at that time , I took the back of the trans off and cleaned and tightened the nut to 250 ft-lb with Green loctite and re-staked it and that went to about 185K when the trans failed (my fault) because I used the wrong oil/lube and trans failed, did not have the 5th gear nut come loose. I found in the Mopar Box , a new NV4500, so I replaced instead of rebuilding it and in the Mopar box it came in I found a 4X5 note card with "5th gear mod" printed on it, so I thought all is good and put the new NV4500 in and that trans went to 240K and 5th gear nut came loose and I have only gone 8 miles since that happened. The mechanics I have talked with say to tighten nut to 400+ ft-lb with green loctite and re-stake it and then put the spline nut keeper on. re assemble and drive on. This will be my daily driver and I have a 2016/17 2500 CTD that I use for towing toy hauler ( 5/6 snowmobiles) about 11k lbs and stays hooked up here all winter and then hauls 4X4 atv and side by sides periodically all summer on smaller trailers So that is where I am at , ready to start this job by first getting the T case off and go from there!

The splines of fifth gear have almost certainly hammered into the smooth surface of the main shaft. The only long-lasting fix is to replace the mainshaft with one that is fully splined under fifth gear and fifth is a very snug fit on those splines. See TDR issues 30, 31 on building and replacing the mainshaft, 33 on lube testing.
 
Quad 4x4 has gears that are made just a few thousandths smaller than the original. The aftermarket full spline shafts are junk and still won’t hold the gear without lots of loctite . Quad 4c4 had decent full splined main shafts but they are not cheap . One thing to make sure is that gear still fits with a little bit of resistance or you can get it crooked and the end result will not be something you’ll be happy with . I have the best luck with green loctite and the super nut from all sates gear
