Engine/Transmission (1998.5 - 2002) gear noise in NV4500 Transmission

My friend just had his NV4500 transmission rebuilt and has a low pitch noise at 60-65 mph in 5th gear. It is much more pronounced in 5th, but can faintly be heard in 4th, and goes away at higher or lower speed. Can anyone out there help?

NV4500 noise

Have you ever lost 5th gear? My 97 has 135k, and has hauled a lot of heavy loads. It has a throttle plate, 4" exhaust and an AFE intake. The 5th gear began making a whine, a long time ago, changed the transmission fluid, to Redline, but not much change. The local transmission dude said, no hurry to repair, it is wear!! So, maybe they did not rebuild completely on the upper gear bearings??
