Engine/Transmission (1998.5 - 2002) Hard starting and heavy smoking

My buddy just called with his problem. He do have 00 model with 265000 miles and his truck is very hard to start in the morning, only way to start is with gas pedal on the floor. But bigger problem for him is that truck is smoking very bad, so bad that somebody driving behind him call police and he get not so pleasant letter from DMV. He don;t use any fueling box, only what he did on his truck to put 275 injectors 5000 miles ego, nothing else. Any idea ? He don;t have gauges, not sure if turbo build pressure. Could this be problem with temperature sensor or crank position sensor ? Any idea where to start will help. THX
