They start when one's stool is too hard and dry, which causes the surface tissue to 'separate' a little. In time, they become little sacs of skin hanging in and around one's rectum. They can bleed and be painful.

The best way to avoid them is to eat enough fibre (one should eat a fairly balanced diet anyway) and drink enough fluid (Irish Stout is not a good fluid to drink lots of for this purpose) to keeps your stool ... soft. And don't sit on the porcelain throne any longer than you need to; as someone else posted, that, too, causes problems.
