Injector Problem - Intermittent Return Flow Light, Now Fuel in Oil

Only way I can see that happening installed in the truck is someone left a socket or wrench in there and started the truck. That much gouging should have made a very noticeable noise and if the rockers got that far out it not run right.
 
I don't know about chafing, the damage looks like chipped glass or porcelain, like the metal shattered almost. The valve train looked perfectly fine when I was running the engine with the valve cover off to check for injector leaks, but I was planning on checking the valve lash on all of the cylinders while I have it all apart anyhow.

If it would be chipped, then the injector body would be intact.
Cerbs theory with the forgotten tool could be very possible.
That thing didn't happen within a couple minutes, that needs more time.
Check all and everything very thoroughly.

Would you think that can happen while using the exhaust brake, that the valve stays slightly open and the rocker arm looses contact to bridge?
And pops back into its seat under load?

I just try to figure out how that could have happened.
 
The bridges cannot move that far and not come flying out of there and if they get off a little they will fall out and then it really sounds bad. I think something got jammed between the spring and injector and wore it like that. Maybe it even broke the FOD and washed it down into a low lying area where it is hard to see or find.

Yeah, IDK, I am stumped... All I know is it didn't happen when I've been in there. I am insanely ocd at making certain that nothing gets into the valve train that shouldn't be there. I must check 3-4 times with a flashlight and a magnet before I put the valve cover back on.

All I know is that every time I've removed the valve cover and rocker box, I triple check everything thoroughly before putting it back together.
