Only way I can see that happening installed in the truck is someone left a socket or wrench in there and started the truck. That much gouging should have made a very noticeable noise and if the rockers got that far out it not run right.
I don't know about chafing, the damage looks like chipped glass or porcelain, like the metal shattered almost. The valve train looked perfectly fine when I was running the engine with the valve cover off to check for injector leaks, but I was planning on checking the valve lash on all of the cylinders while I have it all apart anyhow.

Yeah, IDK, I am stumped... All I know is it didn't happen when I've been in there. I am insanely ocd at making certain that nothing gets into the valve train that shouldn't be there. All I know is that every time I've removed the valve cover and rocker box, I triple check everything thoroughly before putting it back together.The bridges cannot move that far and not come flying out of there and if they get off a little they will fall out and then it really sounds bad. I think something got jammed between the spring and injector and wore it like that. Maybe it even broke the FOD and washed it down into a low lying area where it is hard to see or find.
