it is likely that the squashed pushrod is indicating other damage. I would be checking compression on that cylinder because you may have bent valves.
 
I decided to give the alfaOBD app a try since I have the Bluetooth device. I did a cylinder performance test and something is definitely going on with #5.
I was also able to do a injector kill test and guess what, killed #5 and no more knock. Stopped the test and knock knock knock...
I wonder if I got a cp3 issue or a pcm issue. #5 injector is the same one that fried my brand new motor from over fueling

CP3 fuels the rail equally so not sure why that would be linked to the #5 performance? Could be a bad solenoid on #5 hanging or sticking?
 
Now I'm getting a P0606 engine code. PCM could be on its way out. Might explain why #5 injector has been giving me issues since day 1.
Truck has been sitting since 2015 before I took it and put a new motor in it
 
Today I swapped injectors 5 with 4 and the issue is still with #5 cylinder. Did an injector kill test and it stops knocking when I kill #5. I'm pretty sure I got a dropped valve or something. When I started the truck after swapping injectors, I heard something slapping the piston for about a second then it went away. I also get a slight white smoke on idle, not a lot but I can notice it. If I rev it up it puffs a little bit more with black.
Looks like I'll be yanking the head off of this thing yet again. Maybe one day I'll get this damn thing running correctly.
 
With the damage to the push rod do you have access to a bore scope in order to check the condition of the tappet.
 
Did some tests today. Initially #5 had 200 psi of compression. I decided to mess with the valve lash as I was noticing that #5 exhaust was not closing properly, the rocker arm was always under tension. After making sure the lash was set to .010 intake and .020 exhaust. I was able to get 350 psi. I did not have the right adapter to do a leak down test so I couldn't test if a valve was leaking. Put the injector back in and it's still knocking on #5.
Last year when I did my compression test I was over 450. I only had the time to do the test on #5 today but I think I got a leaky or burned up valve.
 
I'd pull the head.......also, don't get fooled with the P0606. That one will pop up after changing injectors. All I could think up was an injector solenoid being in a bind from the connector tubes being too tight. Loosen all 6 then re-tighten. Todd from TC diesel guided me through my madness last year when a friends 5.9 decided to drink a bunch of water.

Yeah I'm not too concerned with the P0606 code. It only comes up when I'm using the AlfaOBD app.
I let it warm up and checked for blow by, it's not bouncing the cap around at all and very minimal smoke coming out of oil fill cap. No smoke coming out of dump tube going to the ground. No crazy smoke on exhaust
I'm going to do a leak down test before pulling the head. I want to be able to pinpoint if it is a valve issue

Bore scope the cam lobes for that cylinder as well.........I doubt thats your problem but you can rule it out easily.
