The pdf listings are the specs for the medium-duty ISBe that any truck manufacturer can buy (including Ford for the F650/750). The current 5. 9L ISBe can also have either front or rear gear train; Dodge opts for front for the Ram application.

Why is the rear gear quieter?



It would be awesome if Dodge opted for the rear gear train; it suggests the possibility of attaching the air compressor that is typically available with rear-gear Cummins engines!



But I doubt Dodge would opt for the rear gear version.

why is rear gear train quieter?

The following is my guess: the flywheel keeps the back of the crankshaft turning at relatively constant angular velocity so torsional oscillation of the crankshaft will make angular velocity at the front more variable; with the timing gearing at the front the velocity oscillations would have more chance of making meshing gear teeth separate and then recontact with a clack.

Engine bay is too small as it is; can barelyget to the two back cylinders now. Dodge needs to realize a straight siz is a long engine... hopefully the next body will accomodate the Cummins better (aka longer nose).
