I think the secret is to put the nozzle all the way into the filler pipe to open the second flapper down it there. As compared to my 2003, the filler "pipe" is a much longer formed steel tubing that necks down and extends all the way over to the frame rail before it turns into rubber hose. The steel vent line taps off the larger section of the filler pipe attached to the truck fender and also extends over to the frame rail before it changes to hose. I have some pictures that I took while reinstalling my transfer flow from under my 2013 truck but I don't know how to upload them onto the TDR website. I guess you can tell I figured it out. These show how the DEF and fuel fill are connected under the truck. You can see that I had to cut a section of the vent line steel tubing out to be able to insert the Y fitting for the Transfer Flow. Care is required due to the fact that the DEF lines are plastic. Ken Irwin

Ken is right, you need to insert the nozzle in as far as it will go to open the second flap. B*lls deep if you will :-laf. You can't however, clip the handle and walk away anymore.
 
Thanks for the detailed pictures and explanations. I had not considered nor yet seen the filler for the DEF and the maze of hoses under there just make me even more ticked off at the greenies. I have a hell of a time filling my BMW R1200 RT with gas due to the evap gas filling hoses. Unless I use an extension gadget ( a McCuff) I bought for that purpose you cannot get a full tank without getting gas on your hands or all over the tank. I have to hold the hose and lever now for our SUV so I guess I could do the same for the diesel. But why should we have to? With cooler weather coming, I am looking forward to examining the new trucks in a lot more detail. The last time at a dealer it was about 130-140 degrees on the pavement and unless they were sealing ice cubes I was not interested.
