This is what I have done so far, one front side mostly done. I tacked everything and need a more competent welder to finish the welds. This is with a 3" lift built in, for my 2006.
The plate by itself with the 3 holes is what sits on the spring bracket in place of the shock tower. It will have an upper shock bracket welded on.
The lower ring sits in the coil spring cup and will bolt to the lower shock mount with two tabs not yet installed.
I am still trying to get ride-height lengths of shocks so I know how far to space the upper and lower shock brackets but... Guess I will have to measure my stock ones and add 3" for front and 2" for the rear.
