Engine/Transmission (1994 - 1998) Hard Start Situation

I have a 98 2500, 4x4, TST #11, GSK, 13" South Bend Clutch and I lost 5th gear this morning driving on a flat section of highway, not towing, doing about 70mph. No bang, pop, skip or anything. I'm assuming it's the nut backing off on the main shaft. I'd really appreciate some insite here. Can I access the nut from the back of the transmission after removing the transfer case but without dropping the transmission? Is there an upgraded nut available? Any help here is greatly appreciated!

It is on the rear of trans case. On 2wds, we remove the tail housing for access. There is a new upgraded nut, but on mine I just cleaned things with brake cleaner and reinstalled, banging the nut real tight. Has worked for 80k or so.
 
you can also get a new / improved nut from Quad 4x4. quad4x4.com
good stuff. . If the spline isn't damaged, go ahead and replace the old nut with the new/improved one... good luck.
