I have a stock '03 2500 with 302,000 miles. Just replaced the injectors due to rough engine performance. Stalling, variable idling(would drop to 500 rpm almost stall then spike to 1200 rpm - truck shakes and blows a puff of black smoke) jerky starts from a stopped position. Still runs rough.
 
I would assume you have checked the intake system for blockage, that is reasonably clean air filter and the turbo is in good shape.



New fuel filter?



Sorry, I'm not an expert, just tossing out some ideas.
 
I just had my injectors changed on an 06 for rough idle. I was hoping that would be the fix but no such luck. Looking for any thoghts. Maybe adj. valves or a bad FCA. It does seem to be a little better with the smarty on level 3 but it is still there.

our 03 was in the shop and replaced all 6 injectors due to same problems- before the injectors we went through 3 FCA's, fuel pump and pressure relief valve in last 6 months, picked it up today and while it was there the mechanic found some algae growing in the fuel. We buy all our fuel at a commercial place. He got it cleaned out put new filters and something called Kill in the tank. We got within about 10 miles from home and it started acting up again. We called him and he said to change the fuel filters and see what it does. Hope thats it and not the expensive fuel pump.
 
Adjusted valves today. Still have a rough idle but it runs noticeably quieter. I guess the next thing I will try is an FCA. It is available at the local Cummins dealer for $102.

Just an update, the shop who replaced the injectors took it back, did all the pressure tests possible, spent some time with Bosch and figured out it was a problem with an injector body. Problem has been fixed and truck is stronger than ever1
 
Took my truck back to the dealer this morning for rough idle (still). After talking some with the mechanic his thoughts were maybe a bad fca or programing issue. Stopped in about 1 to see how it was looking and :eek: to my surprise they had the top end tore apart. He was telling me that there was some blow by and that it looked like the rings might be broke on #6 that there was a grove in the cylinder wall. H was wanting to do an in frame rebuild but hoping they put a short block instead. Praying this is the fix if not might be looking for a new rig.
 
Any one have the part number for the FCA and is there a on line parts dealer that has them? My rig has started bucking when coasting down hill while in cruise controll also has slight stumble at idle which clears up when I do the outboard oil in fuel test.
