Will start if I bump starter and wait about 20 sec. Seems like air in line, so did vacuum check on whole fuel system, and all is good. Installed new lift pump. Didn't help. Any ideas. Thanks.
 
Yes, purged air. Truck does run if I wait the 20 sec. Sorry I didn't make that clear before. I just now discovered that I don't have to bump the starter. I just need to turn on the ignition and wait the 20 sec. So i am sure it is not a fuel problem. Something electrical. Have checked all connections.
 
Will fire right up, cold or at operating temp, as long as I wait the 20 sec. after turning on the key, then engage the starter.

I could be that either one of your connector tubes or injection lines isn't seated properly. I put a set of RV275's in a buddy's truck and his lines and connector tubes had quite a bit of crap on them. Well I didn't clean it off and he had similar results to what your experiencing. I cleaned and polished (1200grit sandpaper) the connector tubes and tips of the injection lines and it cured everything.
 
My baby is all better!!! Problem seemed to be a 20 sec delay from the CPU to the fuel injector fueling solenoid after turning on key.

A TSB 18-014-01 flash cured the problem. I will now be able to get a good nights sleep. Thanks for the suggestions.
