I have a 93 D350 and I have dont know of any one else who has had this happen so i'll put it out there for the rest of you to think about. The press in oil plug located in the lifter gallery just behind the cover plate came out of its bore dumping the oil pressure and pushing the oil out the crank case vent. With the loss of the oil the engine ate its bearings and all six pistons, but the block and crank were ok. Before rebuilding it i had the oil passage tapped and pipe plug put in so it cant push out again.
 
Sounds like an anomoly. I've never heard of this before. Maybe the loctite bottle went dry when they were assembling that engine. Doo doo happens.

Unfortunate for you though.
 
I really dont know why it can out but when i took off the lifter cover plate the plug was out of its bore and back by the crank case vent. I didnt see any thing that looked like loctite left in the bore or on the plug maybe there wasnt any there maybe there was, as far as corrosion the bore and plug were both clean. I think this is one of those things that goes up as crap happens and was just bad luck.
