Ok, I have an odd question. I now drive a 97 3500 2x4, 5 speed, 4:10 rear end. I also have an 86 GMC 2 1/2 with 350 with a 5 speed and an over/under. So my question is will that over/under work behind my truck or will I have to just shell out the cash and buy one?

It should work. Most of the Brownies were pretty heavy duty even the small ones. What are the ratios 20% under and 15% over? I had one in a 1 ton utility truck behind a 4 spd.

do you think that they're will be many mods need to make the old one fit on my nv 4500? i have no idea what type of trans. is in the old truck. again i know about the driveshaft, but im not sure if i will have to come up with something to mate with my trans?

You have to build a couple of crossmembers to hang it from and drive shafts. Shift linkage. depending how mechanical you are it can go from a job to a real pain in the rear. You can take the flanges to a drive shaft shop and they can tell you what u-joints will fit. I would mount it about where the center support is now.
