I'm driving a 5 speed '97 with 145,000 miles on it. I tow a 32 foot 5th wheel race car trailer all over the country. A buddy of mine suggested I go ahead and have the bearings replaced in the rear axle (including limited slip clutch packs) as a preventative maintence item. I have changed the diff. oil every year since I bought the truck brand new. And I've used Redline synthetic every time. I agree that the limited slip doesn't work as well as it use to, but should I really need to change out the bearings at this mileage?
 
I don't know if I would worry about the bearing for the ring and pinion but the hub bearings are cheap and pretty easy to do. It may be worth it if you have a couple free hours someday but I wouldn't make it a top priority IMHO. If you were lacking on your oil changes in your rear diff, as many do, or were using a cheapo gear oil maybe I would consider it more of a priority. JMHO.
