Take a look at my gallery... I have a bumper that might work for you.
There are no drawings for it, but here are the details:
8"x4"x1/4" rectangular tubing that is tapered to 6" on the outboard edges. 1/4" steel plate welded to the back in strategic places to mount to the frame both on the sides and between the receiver hitch and frame. Cut a box big enough to put your license plate in and drill holes to mount lights.
From there you can drill a hole on the right or left side and put a swing out hinge in and fab up a tire holder. Seen it done on a similar bumper, but mine clearly does not have it.
