Over the years I have replaced both parking brake cables twice, the hardware in the drums, and the cable equalizer.

Because you have replaced all of these parts, I would fully release the park brake and back off the cable adjuster along the frame. Then I would remove both drums. Before doing anything else, I would inspect each side ensuring that the brake shoes are seated against the anchor pin. This is an important step and can easily be overlooked.

Sometimes when the cable along the frame has been adjusted out of sequence, the park brake shoes may be seated properly on one side by pushing them to the anchor pin, but the shoes may be pulled away from the anchor pin on the other side via the transfer of energy through the equalizer cable. This is what you are looking for. Otherwise, @Ozymandias has a good procedure to follow.

Do you back off the cable adjustment first?

Nope, the "adjuster" stays where it is. If it was never touched before it will have just the right amount of slack that the shoes can return fully back to the mechanical end stop.

Because it isn't really an adjuster, it is more of a pre-tensioner to fully seat the cables when they are new. Afterwards it shouldn't be used for adjustment anymore and solely adjusted within the drums.
If your setup isn't factory anymore you might want to do a reset/check for the right setting of the cable adjuster, but this can only be done after the drums are set perfectly.

To understand how the setting of the cable assembly is achieved one needs to read that chapter in the FSM at least three times in a row as it sounds so odd what to do.
