I have a 94 2500, pretty much stock. I have 160000 on it. I had to do the front pads, an injector pump (cold starting at -20 without heat) and tires (I have over 70000 on the Mich ATX tires) since I bought it at 68000. My fuel gauge hasn't worked for a few years but I have the part. I believe that other than normal maintenance, fuel, oil, tires you should be fairly well off. Any other item would be an exception and is hard to budget for. But you must remember that things on these engines are not cheap, so you need to have a fund set aside. Its cheaper to maintain then to repair. Keep good fuel and oil in it and it will be kind to you.
