Well, it's been getting worse for a couple of months now. 58 - 63 mph, only in 5th..... shift to 4th at same road speed and shudder goes away. The shudder will almost loosen you fillings! Because it only happens in 5th (not neutral or 4th), I assume it has to be the transmission and not a tire, U-joint, or carrier bearing.

Am I about to lose my transmission? I've already had the 5th gear nut replaced at about 80K, now at 115K.

I realize that I tow heavy all the time, and am putting a lot of power through the transmission (approx 375 - 400 hp).

Are there any similar experiences out there?

It's possible that 5th gear is backing off. I had the MOPAR 5th gear nut fix installed at 60,000 miles (hadn't lost 5th gear at that point, was just doing it as preventative maintenance) and at just over 100,000 miles I lost 5th gear.
