My 200 ram 2500 has a problem keeping the bely on it. Every 20,000 miles it gets chewed up. i have replaced everything and it still does it. What do i do.
 
Usually the pivot in the tensioner is worn causing the tensioner to twist slightly and push the belt to the front or back. Try a new complete tensioner assembly, not just the pulley.

I replaced the tensioner w/cummins part and it still crept into the A/C compressor one rib at a time. Using a straight edge I checked alignment on the pullys and found that the idler was leaning in at the bottom ever so slightly, just enough to allow the belt to creep off the A/C pully into the compressor. Solution: I loosened up the bolt securing the tensioner and slid (from the bottom up) a 0. 020" feeler gauge upto the underside of the bolt. Tighented it back up and re-checked with the straight edge - perfect for the last 33k. Go figure? It worked for me.
