You never told us how quick your water temp comes up and where it rides.
It takes at least 10 minutes for the needle to move up past 140 (even if she's been plugged in overnight). Temp typically hovers just below 200 on the gauge once she's all warmed up.
 
AFAIK, hi idle wasn't available till '05. Mine won't do it. The only idle up is below a certain ambient temp.

that's not correct. my early 04 has high idle using the cruise controls.


don't turn the fan on high right away either, keep it on low, you might have a partially plugged heater core, not letting enough coolant through, once at operating temp, take a heat gun and check both hoses to the heater core.

Do you know if your truck had ever been flashed or had software updated? Mines original- never updated or plugged into any box. There's no provision for idle up with the DRB.
Being here on the forum at that time, the 03-04's wouldn't do it.

New headlights, fog lights, tow mirrors, and nerf bars. She's starting to shape up.

Anything built on or after Dec 10 2003 has the idle up feature available in the software. The early 04's and 03's need and ECM upgraded to handle the flash that has the PTO option enabled and controllable. Availability is predicated on ECM SN range.

According to the TSB anything back to 98.5 with the correct ECM and control circuit can do the PTO function.
 
Diesels don't generate the heat at idle like a gasoline engine does. Prolonged cold idle isn't going to do the engine any good. Best way to build heat in the engine is driving it right away easy until it comes up to temp. The other way and what I do is apply my exhaust brake and set the fast idle. My truck has the PTO fast idle feature. Cold idle, my EGT will be below 300 degrees. With fast idle and my Pacbrake engaged, I get 575 degrees EGT and a fast warm up.
