After I've been driving for over an hour and stop the truck it has trouble starting again. When I crank the starter it fires, then stops Immediately. I've noticed that if I pump the throttle while cranking it will start. The truck has 165k on it... does this mean its time to rebuild the injection pump, or is it more of a fuel priming problem?

You might want to check your fuel valve relay or the fuel shutoff valve. The relay is pretty cheap and sometimes goes out.
 
The key point here is that it starts right up and then dies. Mine did the exact same thing at almost the same mileage. Check the rubber boot on the fuel shut-off solenoid. Mine had deteriorated and pieces of it were stuck in the return spring. The solenoid has two coils. The "pull-up coil" had enough power to overcome the extra resistance and open the solenoid but the "hold coil" could not keep it open. I cleaned out the pieces of rubber boot, put some anti-seize on the plunger and let her rip. Been doing fine for a year now. Some people put a homemade boot back on but I haven't seen the need myself.

I took a look at my boot... it's really coming apart. I'm going to remove it tonight and I'll post the results after I get a chance to drive it for a while. My mechanic also replaced some kind of banjo bolt that has a spring in it... goes on the injector pump somewhere... he said when the spring gets week it can cause similar problems.
