I started the engine and heard a different noise. I popped the hood and saw and smelled what appeared to be smoke coming from the starter area. I shut the engine off but the starter continued to operate but the engine did not turn over. I had to disconnect the batteries to get it to stop. Is this a starter only issue or are there other issues causing this? The truck is a 1999 and has 225K on the original starter.
 
I believe you are totally correct, new contacts. I had already heard the horror stories and installed the replacements by 30k.

I did them proactively on my 97 as well. Guess it's time to start thinking about them on my 06.
 
Bet the new starter still has same lugs as original. If you still have the original starter consider getting the kit from Geno's and install. I did mine about 8 or 9 years ago and it always works like a champ. Just wish I could have waited for parts to rebuild the original alternator, but it was failing during bad weather and time was not on my side. I had to return core (original) or pay ridiculous core charge...
 
I hope you didn't choose a cheap parts store rebuild. If you did you will be doing the job again.

An OEM Denso starter with the HD contacts/solenoid pack will be the most reliable starter for these trucks.
